Iliopsoas Impingement: A Newly Identified Cause of Labral Pathology in the Hip

Purpose:

To describe iliopsoas impingement as a newly recognized cause of anterior labral tears near the iliopsoas tendon in the absence of bony abnormalities.

Methods:

Retrospective review of 25 patients treated with isolated iliopsoas tendon release and labral repair or debridement; minimum 1-year follow-up.

Key Findings:

  • Labral tears adjacent to the iliopsoas tendon occur due to compression or traction by the tendon.
  • Surgical release of the iliopsoas tendon combined with labral treatment improves hip pain and function.
  • Patients showed good to excellent outcomes at an average of 21 months follow-up.

Conclusion:

Iliopsoas impingement is a distinct cause of anterior labral tears, effectively treated by releasing the iliopsoas tendon and repairing the labrum.

In patients presenting with anterior hip pain and labral pathology without clear bony abnormalities, consider iliopsoas impingement as a potential etiology. Labral tears adjacent to the iliopsoas tendon may result from mechanical conflict. Surgical management with iliopsoas tendon release and labral repair can yield favorable outcomes in appropriately selected cases.
