Purpose

To assess prevalence of intra-articular hip pathology in younger retired NFL players evaluated for persistent hip pain.

Methods

Retrospective review of 62 hips in 38 retired NFL players undergoing hip MRI or MR arthrogram. Images analyzed for labral tears, chondral lesions, ligamentum teres tears, cysts, osteophytes, and player history.

Key Findings

  • 89% had labral tears, 98% had chondral lesions, 81% had ligamentum teres tears.
  • Osteophytes (5%), subchondral cysts (14.5%), and paralabral cysts (5%) were also noted.
  • Most had bilateral symptoms.
  • Average alpha angle was 59°, indicating prevalent cam deformities.

Conclusion

Younger retired NFL players frequently have significant intra-articular hip pathology with common bilateral symptoms.

What Does This Mean for Providers?

  • Clinicians should maintain a high index of suspicion for labral and chondral damage in symptomatic retired NFL players.
  • Comprehensive imaging evaluation is warranted for accurate diagnosis and treatment planning.
  • Early identification of pathology may improve management and outcomes in this high-risk population.
