Patients With Concomitant Painful External Snapping Hip and Femoroacetabular Impingement Syndromes Report Complete Snapping Resolution With Release of the Gluteus Maximus and Iliotibial Band, and Comparable Minimum 2-Year Outcomes to a Propensity-Matched Control Group

Background:

ESH and FAIS are common coexisting conditions causing hip pain and dysfunction. This study evaluates outcomes in patients treated arthroscopically for FAIS and painful ESH, including release of gluteus maximus and iliotibial band.

Methods:

Prospective data collected on patients with both FAIS and painful ESH undergoing hip arthroscopy with ESH release. Outcomes compared to matched FAIS-only controls.

Key Findings:

  • 100% resolution of snapping hip symptoms in ESH group.
  • Significant improvement in mHHS, NAHS, HOS-SSS, and VAS at minimum 2-year follow-up.
  • Outcomes were comparable between ESH plus FAIS group and FAIS-only controls.

Conclusions:

Release of gluteus maximus and iliotibial band during hip arthroscopy in patients with concomitant ESH and FAIS yields complete snapping resolution and comparable functional outcomes.

For patients presenting with both FAIS and painful external snapping hip, simultaneous treatment of ESH with gluteus maximus and iliotibial band release during arthroscopy is effective. Providers should assess for snapping hip in FAIS patients and consider incorporating this release to optimize patient outcomes.
