Purpose

To compare clinical outcomes between arthroscopic labral reconstruction and segmental resection in patients with irreparable labral tears associated with femoroacetabular impingement (FAI).

Methods

A matched-pair controlled cohort study including 11 patients undergoing labral reconstruction versus 22 patients undergoing segmental resection. Patient-reported outcomes were assessed at a minimum 2-year follow-up.

Key Findings

  • Labral reconstruction patients demonstrated significantly greater improvements in validated functional scores (NAHS, HOS-ADL).
  • Both groups showed postoperative improvement, but reconstruction provided superior functional outcomes.

Conclusion

Arthroscopic labral reconstruction should be preferred over segmental resection in managing irreparable labral tears to optimize functional recovery and pain relief in FAI patients.

What Does This Mean for Providers?

  • Consider labral reconstruction as the treatment of choice for irreparable labral tears to improve short-term hip function and patient outcomes.
  • Segmental resection, while effective, may result in comparatively inferior functional recovery.
  • Surgical planning should prioritize tissue preservation and restoration when feasible, supporting evolving standards in hip preservation surgery.
