Arthroscopic Treatment of Labral Tears in Patients Aged 60 Years or Older

Background

Hip arthroscopy is standard for labral tears in younger patients; however, its efficacy in patients aged 65+ is less defined.

Methods

Multicenter study of 26 patients aged ≥65 undergoing arthroscopic labral tear treatment, evaluating outcomes via Hip Outcome Score (HOS) and modified Harris Hip Score (mHHS) over at least 2 years.

Key Findings

  • Significant functional improvements were observed postoperatively.
  • 9 patients required total hip arthroplasty during follow-up.
  • Average patient satisfaction was moderately high (7.7/10).

Conclusions

Arthroscopic labral treatment can offer modest improvement in elderly patients, but the risk for subsequent hip replacement remains notable.

Exercise careful patient selection when considering hip arthroscopy in older adults. Counsel patients about the realistic benefits and the potential need for eventual hip replacement. Consider arthroscopy primarily for those with minimal arthritis and good functional goals.
