Durable Mid-Term Outcomes and High Rates of Meaningful Improvement After Hip Arthroscopy With Concomitant Periacetabular Osteotomy

Background
In patients with acetabular dysplasia and concomitant intra-articular pathology, combined PAO and hip arthroscopy is increasingly utilized. Mid-term durability and clinically meaningful outcome attainment are critical for assessing efficacy.

Methods
Retrospective review of a prospectively maintained cohort of patients undergoing combined hip arthroscopy and PAO with minimum 5-year follow-up. Outcomes included mHHS, NAHS, HOS-SSS, iHOT-12, MCID, PASS, SCB, and THA conversion rates.

Key Findings
Significant improvement across all PROs at 5-year follow-up with no deterioration between 2 and 5 years. Sustained high rates of MCID, PASS, and SCB achievement. Low conversion to THA (5.7%), indicating durable mid-term survivorship.

Conclusion
Combined hip arthroscopy and PAO demonstrates durable mid-term outcomes with sustained clinically meaningful improvement and low failure rates, supporting its use in appropriately selected dysplasia patients.
