Background

This study evaluates whether the preoperative lateral-to-medial (L/M) joint space ratio predicts survivorship (avoidance of conversion to hip arthroplasty) after hip arthroscopy for femoroacetabular impingement (FAI).

Methods

  • Retrospective analysis of 1,885 patients undergoing hip arthroscopy.
  • Preoperative radiographic joint space measurements assessed.
  • Minimum 2-year follow-up to determine survivorship.

Key Findings

  • The L/M joint space ratio was the strongest predictor of arthroscopy survivorship.
  • Patients with an L/M ratio ≥0.75 had a 91.7% survival rate vs 75% in those with ratio <0.75.

Conclusions

The L/M joint space ratio is a significant preoperative metric that may guide surgical decision-making and prognosis in hip arthroscopy.

What Does This Mean for Providers

  • Incorporate L/M joint space ratio measurement into preoperative radiographic evaluation to better predict hip arthroscopy outcomes.
  • Use this ratio to counsel patients on their risk of needing future hip arthroplasty.
  • Consider alternative treatment strategies or closer follow-up for patients with L/M ratios below 0.75 due to higher risk of failure.
