Background:

Irreparable abductor tendon tears can result in significant gait disturbance. Gluteus maximus and TFL tendon transfer may offer a salvage solution.

Methods:

Case series of 3 elderly female patients undergoing gluteus maximus and TFL transfer. Assessed gait, strength, pain (VAS), and satisfaction over ~2 years.

Key Findings:

  • 2 of 3 patients achieved normalized gait and strength gains.
  • VAS scores dropped to near zero in responders.
  • One patient had mixed results, underscoring variable outcomes.
  • No complications reported.

What Does This Mean for Providers?

  • Gluteus maximus/TFL transfer is a viable option for non-repairable abductor deficiencies, especially in elderly, low-demand patients.
  • Providers should carefully select candidates and discuss that while pain relief is likely, functional gains may vary.
  • This technique requires specialized surgical expertise and should be performed by experienced hip preservation surgeons.
