Background:
External snapping hip causes painful snapping sensations due to iliotibial band and gluteus maximus tendon issues; traditional surgeries involve altering the IT band, which has drawbacks.
Technique:
Minimally invasive endoscopic release of the gluteus maximus tendon insertion to reduce tension without changing iliotibial band anatomy.
Methods & Results:
- 8 patients (9 hips) treated; most achieved symptom resolution.
- One revision was successfully performed.
- All returned to prior activity levels without nerve or vascular complications.
- Hip function scores improved.
Conclusion:
Endoscopic gluteus maximus tendon release is a safe, effective, and less invasive option for external snapping hip syndrome.
What Does This Mean for Providers?
- This technique offers an alternative to IT band lengthening with fewer anatomical alterations and complications.
- Providers should consider this approach for patients with refractory external snapping hip symptoms.
- Understanding endoscopic anatomy and technique is critical for successful outcomes.
