Background:

Painful internal snapping hip is frequently attributed to iliopsoas impingement, often seen in conjunction with femoroacetabular impingement (FAI) or acetabular dysplasia. Traditional interventions—such as iliopsoas lengthening or release—can alleviate symptoms but may lead to adverse effects, including hip flexor weakness, tendon atrophy, and compromised postoperative function. As a result, there is growing interest in tendon-sparing surgical approaches to manage this pathology while preserving strength and dynamic stability.
